Note that this is an advanced registration intended to provide you a spot in the tournament, you (and your parent / guardian if you are under 16 years old of age) must sign a printed copy of your registration at the advanced registration desk on the day of the tournament in order to participate.

Waiver, Release and Indemnity Agreement:

Release, Waiver and Indemnity: In consideration of the acceptance of my application and the permission to participate in the MAC Open Tournament. I Agree to abide by all the rules of the Thunder Bay Martial Arts Council. I further agree that I will not hold the Thunder Bay Martial Arts Council or any member of the Thunder Bay Martial Arts Council, or facility were tournament is held responsible for any injury that I might sustain as a result of my participation in the MAC Open Tournament. I for myself , my heirs, executors, administrators and successors, release all and any claims against the Thunder Bay Martial Arts Council, and any facility where MAC Open Tournaments are held, arising out of personal injuries sustained while participating in the tournament and while on the premises on which the tournament is held, and all other successors and assigns of and from all claims, demands, damage costs, expenses, actions and causes of actions whether in law or equity, in respect of death, injury, loss whether as a spectator, participant, competitor or otherwise, whether prior to, during or subsequent to the tournament and notwithstanding that the same may have been contributed to or occasioned by the negligence of any of the aforesaid. I further hereby undertake to hold and save harmless and agree to indemnify all of the aforesaid from said tournament. By submitting this application, I acknowledge I have read, understood and agree to the above waiver, release and indemnity. I have studied the rules and regulations by which the MAC Open tournament is to be governed, and I agree to abide thereby. Participation in this Tournament should be considered a high risk sports activity, my participation in this tournament is at my own risk, and further I warrant that I am physically fit to participate in these activities.
